That he is a Vice-President of _______________________ , the holder of the mortgage hereinafter described, which is a lien on premises situated at ______________________ , New York, New York.

That the mortgages held by the undersigned are (1) in the original principal sum of
$____________ , made by ____________________ to ___________________________ and recorded ________ in Reel/Liber ______ at page _____ in the office of the Register/County Clerk, County of New York on which the mortgage tax was paid in the sum of $___________ was paid, and (2) in the original principal sum of $_____________ 2 made by ____________________ to _______________ and recorded in the Office of said Register/County Clerk on _________ in Reel/Liber ______ at page _______ on which mortgage tax was paid in the sum of $_____________ ;

which mortgages were consolidated by Agreement dated ____________ made by and between _____________________ and _____________________ recorded on
___________ in Reel/Liber _______ at page _________ to form a single lien of $_____________, and assigned to _______________________ by assignment dated __________ recorded prior hereto in the office of said Register/County Clerk;

That there is presently owing on said mortgages the principal sum of $______________ .

That ___________________ , the record owner of the said property and the undersigned have entered into a Agreement dated ___________ , which Agreement is intended to be recorded in the said Office of the Register/County Clerk ___________ County simultaneously herewith.

That the Agreement herewith tendered for recording is given for the purpose of further securing the same principal indebtedness and said Agreement does not create or secure any new or further indebtedness or obligation other than the principal indebtedness secured by said mortgage. That there have been no reloans or readvances on said mortgage.

WHEREFORE, deponent respectfully requests that the consolidation and/or extension agreement tendered herewith for recording be declared exempt from taxation pursuant to the provisions of Section 255 of Article 11 of the Tax Law of the State of New York.
