A uniform form of Acknowledgment and a Certificate for a Proof of Execution by a Subscribing Witness must be used on and after September 1, 1999.
A uniform form of Acknowledgment and a Certificate for a Proof of Execution by a Subscribing Witness can be used outside of New York State on instruments affecting real property situated in New York effective January 21, 1999.
